Job Purpose
To work with vulnerable adults to assess their needs and develop support plans to achieve an optimum level of independence and to enable them to develop and maintain relationships and be valued members of their community.

To be involved in the Care Management process under the overall responsibility of a Senior Social Worker or Assistant Team Manager.
