The Education, Health and Care (EHC) assessment and review team are responsible for delivering high quality casework in line with the statutory requirements in the SEND Code of Practice 2015 and Children and Families Act 2014.
Statutory responsibilities include coordinating multi-agency assessments of education, health and care and ensuring that Education, Health and Care Plans are up to date and reflect the needs of the child and the provision they require.

The post-holder will coordinate statutory EHC needs assessments and plan processes.
The Coordinator will be the person who liaises with other professionals to ensure that the child/young person and their family have an organised and coordinated approach in order to meet their outcomes.
The EHC Coordinator will act as the key point of contact for the child, young person and family, and will coordinate activities that are required to assess needs, meet specified outcomes and ensure the provision of holistic care and support.
We are looking for and EHC Coordinator who will facilitate the coordination of roles across education, health and social care.
