Nonstop is currently working with a well established local authority who have a hospital based Social Work role in the Slough area as part of their Hospital Care Management team.
You will be based out of the local hospital, allowing which will allow for a lot less travelling and a more structured day with less Out of Hours being completed.
Responsibilities
The team is made up of experienced social workers and social care assessors who work alongside doctors, nurses, occupational therapists, physiotherapists, mental health services.
To provide housing related support to vulnerable individuals together with providing housing and property management with the aim of achieving a successful move-on and other measurable outcomes including supporting service users to achieve independence.
Key Responsibilities
Plan and deliver an effective support and safety plan with the service user referring to specialist agencies as required.
Engage with actual and potential referral agencies including local authority central access point/gateway services, statutory and voluntary sector agencies in order to generate referrals for the service as required.
Safeguard children in kinship and connected care arrangements, ensuring their protection and well-being through suitable interventions.
Develop and implement personalised support plans aimed at promoting the safety, development, and well-being of children in connected care placements.
Conduct detailed assessments of children's needs within the Connected Carers Team, in accordance with the Children Act 1989 and relevant fostering legislation.
