Key Responsibilities Assess and prioritize incoming emails to Social Care Direct, providing notes and guidance for team action. Distribute emails equitably among team members to ensure efficient workload management. Offer support and advice to team members on email assignments to facilitate informed decision-making. Identify and direct appropriate processes and pathways, including reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge. Screen emails meticulously to determine their suitability for referral to Social Care Direct, ensuring compliance with protocols. Requirements Professional qualifications in Social Work, registered with Social Work England. Experience in social care practice, especially in triaging and directing referrals. Strong organizational skills for managing email correspondence and workload allocation. Excellent communication skills to provide clear guidance and support to the team. Knowledge of adult social care processes and pathways, such as reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge.
