Practice Development Lead Home Based 38 hours per week £37,370pa with £4,224pa for Car Allowance Purpose of Role This role will take a lead on designing and implementing the Active Support strategy across Ambient's services. By equipping our support colleagues with the skills, they need to ensure that people we support can live the life they choose. The role will provide guidance and support to teams across the organisation to ensure that Active Support becomes the standard operating practice model within Ambient. Active Support To design the Active Support Strategy (within the Quality Strategy) and deliver the project. To be the project lead for the roll out of Active Support within Ambient To lead on the implementation of Active Support across Learning Disability and Mental Health Services To ensure Active Support is embedded across services and service delivery reflects the objectives of Active Support To be solution focused and work through any barriers and challenges to ensure the successful role out of Active Support To work with the Quality team to design appropriate functions to capture, monitor & report on people we support's goal achievements & outcomes. To provide induction training programme for all staff Mentor and coach all managers in all things Active Support To work collaboratively with teams across Ambient to ensure Active Support is successfully rolled out and embedded. Practice Lead & Quality To support staff and teams regarding support risk management of service users. To develop and deliver training relating to best practise service provision. To develop, introduce and review effective tools and evidence-based practice to aid our process of supporting people to achieve positive outcomes. To be aware of new and innovative ways of working that is evidence based that may improve Ambient's service delivery. To lead on identifying, sharing, and implementing evidence-based practise through quality improvement and service development. Monitor the use of Restrictive Practises within Ambient and support teams to ensure the reduction of restrictive practises are at the fore front of their service delivery. Provide advice and guidance to teams on practise matters affecting good quality service delivery. Embed reflective practise across the charity. Quality and Service Improvement To work with Ambient Guardians to ensure that we work collaboratively and people we support are involved in organisational change. To provide targeted support and guidance to services that require an extra focus to drive quality improvements. To report on service / people and or organisational compliance or reputational risks and to be solution focused in supporting any necessary service or organisational improvement. To shape audit tools and governance tools in measuring quality of support and the quality of life of people we support To identify opportunities for quality initiatives, leading on the delivery of those in services and/or organisationally Identify any themes & or trends in any quality or practise data (qualitative or quantitative) and from this identify solutions to ensure we continuously improve Ambient service deliver and governance. Where necessary to carry out practise observations, audits and reviews of services and implement changes that may be required from findings. To be involved in tender responses in relation to care, support, evidence-based practise, quality, and outcomes. About you You are a knowledgeable social care leader who utilises the evidence-based practise of Active Support to improve the quality of life for the people we support. You are skilled at coaching and guiding people to ensure you get the best out of them. Your leadership skills facilitate organisational changes to positively impact our colleagues and the people we support. Ideally you are already an Active Support Lead/Coach/Facilitator Qualifications/Education/Training: Experience in project management and leading on change. Relevant Active Support Qualification is essential. Educated to degree or equivalent is desirable. Experience of leading & managing projects Experience in Active Support, coaching and mentoring teams. Experience of working with diverse stakeholders Experience in driving quality improvements. Experience of working in the charitable sector Essential Skills/Knowledge/Attributes: Good knowledge of data management systems Good knowledge and use of Excel Good knowledge and use of Teams/Microsoft Computer literate to a high standard A knowledge of the adult social care sector and the legislation that governs it. Ability to communicate and work across departments to improve the quality across Ambient. Solution focused and work using initiative. Ability to review themes and trends in data & practise observations. Extensive knowledge of good practise in the Learning Disability and Mental Health Sectors Knowledge of local/national priorities/initiatives/innovations that may impact on policy & practise. Positive Behavioural Support (PBS) Practitioner £30,000 - £35,000 per annum Remote-based with regular travel to sites across the Midlands and Yorkshire. Their promise to their service users is to deliver personalised care and support to meet their needs, and as a PBS Practitioner, you will be key to ensuring the support teams are able to contribute to that high standard of care, through the implementation of Positive Behaviour Support. Reporting to the Group Training Manager/PBS Lead Practitioner, you will work flexibly to meet the PBS needs of the individuals they support. As a PBS Practitioner, you will be passionate about improving the quality of life for those living within their services and achieving improved outcomes. You will achieve this through providing high-quality PBS advice, training, and implementation across the tiers of support. You will play a key role in supporting teams to create enabling environments and reducing behaviour-based barriers to progression. You will ensure that the service meets the physical, environmental, social, and emotional wellbeing of each individual person in a way that respects the dignity of the individual and promotes independence and wellbeing. This position will include networking across services to provide PBS support, giving opportunity to develop wider experience. You must be committed to the development of the teams and the homes mission to achieve the PBS quality framework. You will do this by: Providing PBS guidance and support to the services. Providing coaching, training, and mentoring to the teams. Supporting the embedding of Active support, Creating capable environments, Human rights, Trauma informed care, Restraint reduction and Focused based support. Conduct individual assessments, including for those displaying behaviours of concern. Support the teams to Implement PBS and restraint reduction plans. Provide guidance with pro-active strategies of support and risk management plans. Represent the PBS support team as part of MDT meetings, as required. Monitor and analyse behavioural data, and create reports evidencing behavioural incident reduction. Attend and facilitate best practice meetings. Requirement of role Understanding the experiences and needs of those you will be supporting is extremely important. Therefore, the successful candidate will: Have achieved or is working towards a level 4 PBS qualification. Have a minimum 2 years' experience within the Health and Social Care sector. Understanding of MCA/DoLS, Restraint Reduction, Human Rights, creating capable environments, Trauma Informed Care, Positive Behaviour Support and Supporting Behaviours of concern. Excellent interpersonal, prioritisation and communication skills, both verbal and written. Understands the use of functional assessment and intervention. Communicates complex and sensitive information effectively. Have proven experience of working with people who display behaviours of concern. Be willing to train in physical intervention. Is confident with teamwork, leadership and effective communication skills, self- motivation, and the ability to use own initiative, excellent organisational and time management skills. Is willing to travel to service locations group wide. Is well presented, punctual, reliable, and professional at all times. Can maintain appropriate and accurate records of all documentation, making timely submissions of the planning outcomes achieved, and to evaluate all risk management plans. Can make ongoing adjustments and recommendations to ensure the plan of care is of a consistently high standard and that the needs of the individuals are being met, with all risks reduced as far as possible. Can maintain your own continuous professional development (CPD) by attending and participating in identified meetings and learning opportunities, to enhance your own skills and knowledge.
