About the service
To provide tailored support for a young person on request by social services, this would be target specific and would be reviewed on a week to week basis.
The planned support would be to meet the needs of that young person ie, A 6 week healthy living programme where you would target exercise, diet, fluid intake, hours of sleep etc and monitor it weekly.
