We have an exciting opportunity for an Occupational Therapist to join a busy reablement team in Somerset.
This role involves working with the Council supporting hospital discharges and community reablement through a Therapist-led service that incorporates technology-enabled care interventions.
Key responsibilities include caseload management, assessment, and goal setting to develop reablement plans and deliver short-term interventions for up to 6 weeks.
We are currently looking for an Occupational Therapist to carry out community local authority assessments for both standard and specialist equipment, minor and major adaptations.
