This role involves coordinating and implementing care plans for patients, liaising with other healthcare professionals, and ensuring that patients receive the highest standard of care.
To act as a secondary care assessment worker/short term intevention, carrying out assessments of clients referred to CMHT as they are suffering with a severe and enduring mental illness requiring secondary care.
The role is to work as assessment and/or care coordination worker for referrals received into the team from other teams, i.e.
