St Andrew's community services offer supportive transition that are innovative "own front door" single occupancy, bespoke properties, registered under St Andrew's Healthcare. Each Supportive Transition Service (STS) provides an individualised person centred approach for complex individuals who otherwise do not have a route out of hospital. A role at Billing Lodge represents a real opportunity to support individuals through a transition period from hospital. Treatment programmes involve significant periods of time outside and are firmly focused on supporting community reintegration.Location: Northampton Salary: From £22,392 plus enhancements Hours: Full time (37.5 hours) Purpose of Role: To work as part of a team to deliver and maintain high standards of person-centred care, supporting service users' daily activities and community rehabilitation in accordance with individual care plans and Positive Behaviour Support Plans.To deliver a range of clinical and non-clinical care and therapeutic tasks, under the supervision of registered nurses.
