We are looking for an intensive needs Support Worker for the Lincolnshire area, there are various part time positions and night working positions also, the service users are young people with intensive communication needs, we are looking for someone with support worker experience, an enhanced DBS for Adults & children and a car driver who has access to own vehicle with business insurance.
Key responsibilities
Plan and deliver an effective support and safety plan with the service user referring to specialist agencies as required.
Engage with actual and potential referral agencies including local authority central access point/gateway services, statutory and voluntary sector agencies in order to generate referrals for the service as required.
To provide a helpful, professional & efficient contact handling experience to all customers contacting the Customer Service Centre; across all contact channels, using the appropriate telephony system and social care case management system.
25 hours per week -
The shifts are Monday Friday 5:30pm 11:30pm and Sat/Sun 8am 11:30pm.
