Job Title: Children's Social Worker - 14 Leaving Care Team Location: Cramlington, Northumberland Salary: £26 per hour Northumberland Council is looking for a passionate and experienced Children's Social Worker to join our 14 Leaving Care Team in Morpeth. This role is essential in supporting young people transitioning from care to independent living, ensuring they have the necessary guidance and resources to thrive. Responsibilities: Provide support and guidance to young people aged 14 and above as they prepare to leave care and transition to independence. Conduct assessments and develop personalised pathway plans in collaboration with young people, ensuring their views and aspirations are central to the planning process. Offer practical and emotional support, helping young people to access education, training, employment, housing, and health services. Work closely with a range of professionals, including education providers, health services, and housing agencies, to coordinate comprehensive support for young people. Monitor and review progress, adapting plans as necessary to meet the changing needs of young people. Advocate for the rights and entitlements of young people, ensuring their voices are heard and respected. Maintain accurate and up-to-date records and reports in accordance with statutory requirements and best practice guidelines. Requirements: Registered with Social work England A Degree in Social work or Diploma in SW, CQSW, CSS Right to Work in UK Enhanced DBS check, dated within the last 12 months A full UK driving licence Strong knowledge of relevant legislation, policies, and procedures related to children in care.
