Band 5 111/Crisis Mental Health Telephone Response Practitioner
Role Overview
We are currently seeking a Band 5 Registered Professional to fill the position of 111/Crisis Mental Health Telephone Response Practitioner on a two-month locum basis.
To provide support through outreach and street intervention working with support services and agencies to improve pathways for individuals who are street homeless to access suitable accommodation and services.
To work as part of a team providing additional outreach capacity in order to work collaboratively with new or entrenched rough sleepers in Coventry.

Full Time Monday-Friday 09:00-17:00 (6am-2pm once a week)
We are providing a dual service; engaging those we find rough sleeping along with supporting those newly accommodated, removing barriers and increasing opportunities in persistent and creative ways.
Working in and out of city centre locations the team work closely with partner services to support rough sleepers to move on to both interim and settled accommodation.
