We are looking for a dedicated and professional Care Coordinator to join their dynamic team.

To assist customers in their understanding and completion of relevant documents forms, applications and registrations, over the telephone, including where appropriate addressing their special needs (disability, translation, interpretation requirements etc)
To process referrals and apply agreed eligibility criteria.
The role
To deal with telephone, written, emailed enquiries and respond appropriately to customer enquiries, complaints, requests for information and service requests.
Working with individual service users to provide support, advice, guidance, assistance, and counselling, with the aim of enhancing or protecting individual welfare.
Provision of Leaving Care Support Services via Pathway Assessment and Planning Process, including education, training, employment, accommodation, health, personal support, family and social relationships, practical skills and financial assistance.
Provision of services within the statutory requirements of the relevant childcare, mental health and community care legislation, in line with the requirements of the Council's policies and procedures.
Mobile relief caretaker to cover various estates across the borough..
Working flexibly, alone or with other caretakers to provide an excellent caretaker service on estates as circumstances require.
Key responsibilitiesTo carry out cleaning and other estate duties in partnership with the needs of residents and in accordance with management direction.
Work with clients in producing person-centred support plans and ensure that case reviews are carried out, that support plans are implemented, and adequate case records are maintained.
Conduct interviews with prospective clients using a range of specialist assessment and personalised support planning tools to engage clients in the assessment and support planning process.
You, as an Activities and Wellbeing Facilitator, will work with residents to develop and deliver a range of daily activities that reflect individual needs, interests, choices, and preferences, cultivating a person-centred and inclusive environment that is dementia-friendly and supports diverse needs.
In collaboration with the Volunteer Manager, the Activities and Wellbeing Facilitator will match residents with volunteers to support engaging activities and organize recreational outings and events for social and cultural engagement.
