Leaving Care Worker Job Description The London Borough of Ealing are seeking a Leaving Care Worker to join their team. The successful candidate will be responsible for carrying out assessments of social care needs, including full financial assessments, within departmental guidelines. They will work with service users and their carers to agree on care plans that identify appropriate options. The Leaving Care Worker will also be responsible for initiating the provision of services based on agreed care plans and will record all information regarding needs that have not been met through assessment and bring it to the attention of the Team Manager. Key Accountabilities Responsible for an allocated workload, working to deadlines and prioritising work in line with departmental policy and procedure under the direction of a Team Manager or Deputy Team Manager. Participating in a duty rota and undertaking initial assessments of need, responding to emergencies, and answering inquiries from the public and other professionals about services. Undertaking the guidance of a Team Manager to be responsible for supervising contact visits by parents and other involved parties to children in care in a variety of settings. Monitoring Care plans and reviewing individual packages of care within the complete administration, using the appropriate information technology, to enable management to monitor progress of care plans and take action as required. Person Specification Ability to write reports and ensure good record keeping. The ability to hold a caseload of 26-30 care leavers aged 18-24 years of age. Enhanced DBS required. Job Purpose: To carry out social care and joint health assessments. Devise appropriate comprehensive support plans in order to meet identified needs Ability to evaluate and highlight varying needs for the purpose of different types of care support options, traditional care, reablement or supported living Participate in care discussions and multidisciplinary assessments regarding customers To be creative in determining appropriate services to meet varying needs Ability to research both formal and informal services to meet needs To monitor and review packages of care Principal Accountabilities and Responsibilities: Assessing needs of vulnerable adults who have learning disabilities with complex health needs, challenging behaviour and mental health Provide packages of care in line with assessed need, within available resources and according to relevant social care criteria for services; Review and monitoring of Community Care Packages on a planned basis, as agreed. Maintaining good liaison links with internal and external partner organisations to ensure better outcomes for individuals. Respond in emergencies where packages of care are in danger of breaking down also when key worker is not available; Participate in care discussions and multidisciplinary assessments regarding specific clients and community care needs as required. To keep other professionals informed of developments and alert them to situations where a vulnerable adult with a learning disability is at risk of harm or where they require a more comprehensive assessment of need Experience: Experience of working in Social Care Services or similar organisations such as the NHS or Voluntary Sector. Experience of multi - disciplinary agency working within a multi-cultural environment. Experience of having assessed the needs of people, devise reablement/outcomes/goal setting for the purpose of support planning. Experience of working with vulnerable adults with learning disabilities in a social care context, using acquired skills and judgement in order to build professional relationships, communicate effectively, have appropriate professional attitude when representing the council.
