Our 18-year-old client, who has Cerebral Palsy from an injury sustained at birth, is looking for a Support Worker who may share his love of photography, watching and making movies and visits to the arcade. You will be joining a very close supportive care team who help him live his life to the full. The Role/Client A lives with his mum and sister. He is non-verbal, but able to communicate using aids and pictures/prompts. He is a full-time wheelchair user and his house if fully adapted with hoists. Part of this role will be to take A out to places where he can engage his love of movie making and also to join in with his love of arcade games. A loves watching YouTube so you can look forward to sharing a few laughs whilst keeping him company. Visits out to encourage A's love of photography will also be part of the role. You will be working on a 121 basis, however. a member of the family is usually around to assist with his care when breaks are needed. You will receive full training in all aspects of our client's care and be fully supported by his Case Manager and the wider team. About You Due to A's limitations, he can understandably get frustrated so a calm, supportive nature is a must for this role to focus him back on an activity and manage his frustration. You will have had some care experience but more importantly is your aptitude to care. You will have an engaging, client-centred approach following A's care plan and helping him achieve his developmental and rehabilitation goals. A is fed via PEG so experience in this area would be advantageous. You will be kind, positive and reliable and able to build a relationship of trust with our client. You will enjoy taking the client out on activities and for this reason, you would need to be a driver who can drive a manual van, hold a full UK driving license and be over 21 (for insurance purposes). To co-ordinate and oversee the day to day operation of the Assessment Centre, managing sessions to optimise productivity and customer service, meeting and greeting all customers and visitors and working with clinical colleagues to ensure smooth running of assessments across all relevant channels. Non-London: £23,400 London: £25,643 Fully manage each day's appointment sessions Carry out reminder calls to customers for appointments Monitor session progress and backfill appointments where necessary Identify additional support with other sites where necessary in management of the appointment session Work collaboratively with other Assessment Centres, teams, Team Leaders and Health Care Practitioners to ensure cohesion within unit and work flow progression Welcome and greet customers on arrival whether this be Face to Face, via telephone or video link Arrange travel for customers who require assistance in getting to their appointment Provide a professional outstanding service to customers in line with CHDA vision and values Assist customers with completion of forms, including expense claims, inline with COVID19 guidelines Prepare and maintain rooms and equipment to ensure they are ready for the Health Care Practitioner and Customer Complete daily checks to ensure all completed assessments have been closed down and moved on to prevent a delay with the customer journey Work closely with the Team Performance lead to ensure the sessions run smoothly and to time Effectively communicate with external stakeholders such as General Practitioner surgeries, Hospitals, Interpreting services and Department of Work and Pensions Co-ordinate incoming and outgoing post, ensuring All files are checked to ensure they have arrived at the correct AC.
