The role of a Care Coordinator involves working closely with the Local Authority, Health care professional, GP etc.. and other providers.
The ideal candidate will need to be computer literate and able to pick up new systems relatively quickly and project a positive and professional image at all times, assisting Home Care Worker promptly and conducting all business in a friendly, courteous and knowledgeable manner.
Provide a consultative/supervisory service to a staff team operating within a multi-agency environment to provide professional expertise, skills and advice to enable a safe, effective and high quality service to disabled children and young people that meets agreed existing and new policies and practices.
Manage a complex caseload of disabled children and young people aged 0-25 to assess, plan, monitor and review the support/personal assistance needs of service users, working in a person-centred way to ensure that resources are utilised effectively to safeguard and promote the welfare of disabled children and young people.
The post holder should be working in line with the Social Care Capability Framework.
Manage a case load of children in care, conducting assessments, and developing care plans in collaboration with children, families, and carers.
General
The successful candidate will be responsible for providing direct support to children in care, as well as working closely with foster carers, residential staff, and other professionals to ensure the best outcomes for children and young people.

Job purpose - To assess, plan, deliver and evaluate a high personalised standard of residential care which meets the individual needs of residents and the high standards expected at the home.
