About the Role
Service Care Solutions are seeking a dedicated Community Care Officer to join Haringey Council's First Response Team.
You will assist customers in understanding and completing relevant documents, forms, applications, and registrations, ensuring that all interactions are handled with care and professionalism.
In this role, you will be the first point of contact for customers, handling telephone, written, and emailed inquiries, and responding appropriately to requests for information, service requests, and complaints.
The client is supporting individuals and families who have experienced domestic abuse and offers service users the opportunity to move on and live fulfilled, safe lives following the trauma they've experienced.
To use strengths-based approaches to assess, monitor and review the social care needs of service users and informal carers and find appropriate solutions and make appropriate referrals to external agencies within a preventative, enabling and rehabilitation framework and universal services in accordance with the Care Act eligibility criteria.
Take responsibility for managing casework with the supervision of a qualified senior practitioner within an outcome focussed approach and within the teams expected performance targets and outcomes, whilst promoting independence and assessing and managing risk.
To hold an individual caseload of varying degrees of complexity excluding casework that requires the expertise of a qualified professional.
To coordinate and act as 'named officer' to support and advise parents/carers, providers and agencies regarding the Education, Health and Care (EHC) needs assessment and review process.
To work with and follow up young people who are not in employment education and training (NEET) and/or not known to meet local and national targets when required.
Main responsibilities
Write EHC plans and similar documents, ensuring the EHC plan clearly details the child/young person's needs, is outcome focussed and details the special education provision required for each area of need.
Duties include (but not exclusive to): personal care; assisting with mobility needs; medications (as appropriate to role); infection control; supporting residents with dementia, and end of life care; changing bed linen, sorting laundry, safe disposal of clinical waste, manage stock levels and equipment such as wheelchairs.
Delivery of day-to-day holistic care for residents and meeting their unique needs.
Working with volunteers, Wellbeing Coordinator and therapy teams to provide and encourage meaningful activities.
