We are looking for applicants who can support and advise young people leaving care by helping them to find suitable accommodation, assistance in budgeting and spending decisions, education, employment, and training by undertaking a high-quality needs assessment and pathway plans alongside social workers and working with other multi agency partners.
Our client offers person-centred, independent living options across the whole care pathway - including outreach support to access the community, various supported living options and supporting those with complex physical, learning or mental health needs and developmental disorders.
Every person they support has their own tenancy agreement and their own from door - the emphasis is on delivering support that is grounded in the individuals needs, wishes and aspirations of the person receiving it.
Your new roleAs a Support Worker, you will be providing outreach support to access the community, various supported living options and supporting those with complex physical, learning, or mental health needs and developmental disorders.
Your new companyWe are currently working exclusively with a supported living service provider, who support people living in a range of accommodation styles which include individual flats, specialist single-story properties and shared houses.
They are committed to delivering support to service users to suit their aspirational and individual needs.
