The successful candidate will be responsible for a geographical area, owning all the client relationships in that area, managing their clients care requirements by liaising with their GP's, District Nurses, OT's Discharge, pharmacist, family members etc, as well as their team of Care givers and supervisors, and will be required, at a minimum to template and maintain a 4-week call roster.
