Arrange timely move-on for CEYP into settled social and private rented sector accommodation into settled homes.
To provide a bespoke move on casework service supporting households to access agreed move-on options including, but not limited to, support with bidding, qualifying offers, and access to supported accommodation.
Work collaboratively with agencies externally and internally while advocating and negotiating on behalf of CEYP some with multiple and complex needs, to access appropriate services and engage in long-term housing options.
An enhanced DBS will need to be carried out for successful applicants.
Duties
Carry out the design, installation, maintenance, servicing and repair work for the Association's housing stock and external customers in accordance with agreed targets and procedures.
Support those who have been out of work for longer periods might need extra help to move back into employment.
As a Client Advisor on the Restart programme, you will work with adult job seekers who are preparing to move into employment, helping them to identify and address the barriers that make this difficult by providing exemplary case management, including support, advice, advocacy and signposting to relevant services.
The Restart Scheme will break down employment barriers that could be holding people back from finding work.
To use strengths-based approaches to assess, monitor and review the social care needs of service users and informal carers and find appropriate solutions and make appropriate referrals to external agencies within a preventative, enabling and rehabilitation framework and universal services in accordance with the Care Act eligibility criteria.
To hold an individual caseload of varying degrees of complexity excluding casework that requires the expertise of a qualified professional.
Take responsibility for managing casework with the supervision of a qualified senior practitioner within an outcome focussed approach and within the teams expected performance targets and outcomes, whilst promoting independence and assessing and managing risk.
