General
We are seeking a skilled Nursery Assistant or Teaching Assistant with nursery experience to join a large primary school in Swindon and work in their nursery.
Early years qualifications would be welcomed but are not essential for this role.
Our client, Swindon Borough Council is looking for 2 Experienced Social Workers to join their Urgent Response Team and Discharge Hub Team, who support onward discharge planning from the acute hospital or a discharge to assess bed.
Both are busy and fast paced teams and would require a dynamic worker with experience supporting hospital discharge discharge and admission avoidance.
The team are based in the office 2 days a week, but expected to complete urgent visits at short notice.
To undertake assessments of individual and carers needs and risks to encourage independence, provide support and prevent harm.
To undertake statutory intervention where service users are deemed to be at risk, including situations where people may be neglected or abused in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults.
This includes promoting a multi-agency approach by developing and maintaining effective working relationships.
