Job Title: Acute Liaison Nurses Location: Watford General Hospital and Lister Hospital Salary Range: £35,392 with the opportunity to progress to £42,618 per annum (pro rata for part-time) £1000 Welcome Payment (T's & C's Apply) Hours: 37.5 hours a week (full-time) and 15 hours a week (part-time) Contract: Full-time post fixed for 1 year and part-time post permanent Diversity: We are an inclusive employer and value a diverse workforce This is an exciting opportunity to join Hertfordshire's Community Learning Disability Nursing service in Hertfordshire County Council. As a Band 6 Acute Liaison Nurse you will work within the Health Liaison Team supporting people with learning disabilities when they are admitted to a general hospital. Purpose of the role: As an Acute Liaison Nurse, you will be working within the general hospital to offer your learning disability expertise to reduce the risks of poorer outcomes for people with learning disabilities admitted into hospital for emergency or planned admissions. Your skills in adapting and creating the right approach for the individual will help the person with learning disabilities, your clinical colleagues, and paid/family carers to get the best outcome from their hospital admission. You will be working closely with the Safeguarding Teams and Senior colleagues to develop and progress the Hospital Improvement Standards embedded into the Learning Disability Action Plan. You will be working with Junior colleagues in the team and provide leadership and clinical supervision. Challenges and opportunities: We require people who can be creative and flexible to different approaches for different situations as no day as an Acute Liaison nurse is the same. You will need to be able to demonstrate an ability to travel to the hospital sites and HCC office sites in a timely way. Essential criteria: Qualified Registered Nurse (at least 2 year's experience post registration) Some Learning disability experience Ability to travel to alternative sites within reasonable timescales Multi-disciplinary Team working experience Able to cite evidence-based practice Desirable criteria: Car driver, with access to a car Experience as a nurse supervisor Community-based health care experience Knowledge of social model of disability Teaching skills HCC is a great organisation to work for and as a nurse in the service you will have all the benefits of the NHS Agenda for Change terms and conditions and be nurtured to grow in confidence and skills.
