Job description
To assess and prescribe treatment packages for patients with long term conditions, the frail & elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
To contribute in the delivery of high standards of health care to patients within their own homes, Intermediate Care Beds, or a rehabilitation Unit, by providing proactive, holistic case management and care.

Position Overview
We are seeking a dedicated Band 6 Neuro Occupational Therapist to join our Integrated Stroke Early Supported Discharge (ESD) and Community Neuro Rehabilitation (CNRS) Service on a locum basis.
This full-time position offers the opportunity to work with patients recovering from strokes and other neurological conditions, providing essential rehabilitation and support in the community.
Key Responsibilities
