Within Early Help and Family Help, we are looking to appoint an qualified Social Worker to support the current team manager within the Tameside's Edge of Care team as a Practice Manager.
Based within Tameside, our local service is responsible for providing high quality and person centred support to older adults with extra care needs.
Our purpose built local staff team is both experienced and professional and we wish to build upon this with dedicated, warm and hard working individuals who can help us to provide the best level of personal and housing related support that we can to the individuals that we care for.
The service is part of Creative Support, a national not-for-profit organization dedicated to providing focused and responsive care to individuals with a wide range of care requirements.
Hybrid working - 2/3 day split depending on caseloads/meetings etc
The Transitions Planning team supports young people prepare for adulthood, working closely with those people who are important to them, along with statutory services covering health, education, children's social care and the voluntary sector.
Families First (localities) - This is a dedicated Child in Need Service where you will work on a locality basis with children and families that require a social work assessment.
Please note all roles are now hybrid meaning a blend of locality-based and home working
