Long Term Support Officer Tower Hamlets - East London £17.69 per hour PAYE / £22.80 per hour Umbrella Temporary 3 month with the view of extension 35 hours per week Monday to Friday Overview: As part of a statutory social care team, provide a support officer function which includes the assessment and review of those with care and support needs and case management. To ensure that prevention, strengths-based practice, integrated working and person-centred intervention is central to all intervention. To promote the safety and welfare of service users and carers enabling positive outcomes, working in line with legislation, policy and practice guidance . This role will suit final year Social Work student or newly qualified Social Workers who are waiting for their SW registration The role may involve transferring current commissioned care packages to Direct Payments to give our clients choice, control and flexibility over their care provision Duties: Undertakes care act assessments and reassessments in line with strength-based approaches that identify risks, needs and outcomes both in the community and placements. Complete ongoing assessment of risk to service users and carers as appropriate. Supporting those accessing services to be at the centre of decision making over their support arrangements and arranging advocacy as appropriate. Ensuring those who do not meet the eligibility criteria for care and support are provided appropriate information and advice and signposting to appropriate support as relevant and necessary. Undertake referrals to relevant organisations and departments including via checklists for CHC, advocacy or other General case management and urgent responses to service users and carers as appropriate. Undertake non-complex mental capacity assessments and best interest decisions / meetings with line manager support. (Complex cases are where there is severity and seriousness risk of harm and to liberty). Identifies safeguarding concerns by assessing and responding to initial risks and referring these concerns via safeguarding procedures for enquiries as appropriate by registered social workers. Support registered social workers with gathering information as part of safeguarding enquiries as appropriate. Acting as trusted assessor for relevant equipment as appropriate. Attendance and presentations for relevant panels Co working with senior social workers or registered social workers on complex case work as appropriate (Complex cases are where there is severity and seriousness of risk of harm and to liberty). Promoting preventative interventions including reablement, equipment, adaptations, assistive technology and advice and information. Participate in duty rota as required to ensure the safety and well being of service users and carers. Knowledge: Understanding of the role/remit of longer-term support services to promote independence and strength based practice Awareness of the principles of person-centred care. Basic knowledge of conditions that lead to a reduction in people's functioning.
