Our client, Swindon Borough Council is looking for 2 Experienced Social Workers to join their Urgent Response Team and Discharge Hub Team, who support onward discharge planning from the acute hospital or a discharge to assess bed.
The team are based in the office 2 days a week, but expected to complete urgent visits at short notice.
Both are busy and fast paced teams and would require a dynamic worker with experience supporting hospital discharge discharge and admission avoidance.
