ICT Social Worker

This role has a starting salary of £40,929 - £43,735 per annum, based on a 36-hour working week.

We are very excited to be hiring a Social Worker to join our fantastic Waverley Locality Team as our liaison with the local Integrated Care Team (ICT).

This is a great post for developing experience of Multi-Disciplinary Team working and outreach with clients on a wide range of issues. The ICT seeks to bring health and care organisations together to improve people's lives, with strong local partnerships being a driving force in making better long-term Health and Social Care outcomes, for people.

About the Team

Our Waverley Locality Team: Waverley is a supportive, wide-ranging team. Offices based in central picturesque Godalming with parking and nearby direct main transport links. We are a forward-thinking team that promote ongoing learning, career progression.

Our team has a strong emphasis on working together, reflective practice and career development. The team has a great retention rate, with many staff being with us for over 10 years. New starters regularly feedback how friendly, approachable and supportive the team are. The ICT is based in Farnham Community Hospital and hybrid working is available. Office facilities are also available in Godalming.

About the Role

On a daily basis, you will be meeting with clients to understand their care and needs, how they can be supported and to provide the best outcomes for them. Your work will include but is not limited to:

  • Triaging referrals from GP/Health Community services.
  • Visiting clients in the community.
  • Working with clients to consider Continuing Health Care (CHC).
  • Linking into community services.
  • Joint working with district nurses, dementia link worker, GP practices in integrated Support Planning.
  • Weekly ICT meeting with health colleagues to discuss clients.

You will work alongside a ICT liaison Senior Social Care Assistant and as part of the locality duty team where you will also be undertaking Care Act assessments, Carers assessments and safeguarding work.

Shortlisting Criteria

To be considered for shortlisting for this position, your application will clearly evidence the following:

  • Completion of your Assessed and Supported Year in Employment (ASYE)
  • Registration with Social Work England (We recognise there will be exceptions to this so please speak to us if you are in any doubt)

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area. Reasonable adjustments where needed will be made for successful applicants who have a disability or long term health condition to enable them to fulfil the requirements of the job.
