A London-based Local Authority is seeking an experienced Social Worker to join their specialist Dual Diagnosis team, focusing on supporting adults who are affected by substance misuse and co-occurring mental health issues. **Key Responsibilities:

  • Dual Diagnosis Specialism: Provide targeted support to adults dealing with both substance misuse and mental health issues, conducting comprehensive assessments, care planning, and interventions in line with the Care Act 2014 and Mental Health Act 1983.
  • Multi-Disciplinary Collaboration: Work within a dynamic multi-disciplinary team that includes health professionals such as nurses, recovery workers, occupational therapists, and psychologists. You will collaborate closely with these professionals to ensure a holistic and integrated approach to service delivery.
  • Person-Centred Care Planning: Develop and implement personalised care plans that address both the substance misuse and mental health needs of your service users. Your role will involve coordinating care, reviewing progress, and adjusting plans as necessary to meet the evolving needs of your clients.
  • Safeguarding Adults: Apply your knowledge of the Safeguarding Adults framework to identify and address potential risks, ensuring that all interventions are designed to protect vulnerable adults and promote their safety and well-being.
