Middlesbrough Borough Council are currently looking for a social worker to join their Children's Assessment Team, the post is a full time position with a minimum of 3 months
Requirements
- Conducting assessments of children and families to determine their needs and any risks involved.
- Collaborating with other professionals, including health workers and educators, to gather relevant information.
- Developing and implementing care plans to address identified needs.
- Providing support and guidance to families to help them achieve positive outcomes.
- Participating in case conferences and meetings to ensure a coordinated approach to service delivery.
- Maintaining accurate and up-to-date records in compliance with statutory requirements.
