Hours: 9am-5:30pm
Term: Temporary until Feb 2024 (with the possibility of extension)
Salary: 17.54 PAYE hour

Our client, a dynamic and forward-thinking organisation, is seeking a dedicated On Street Support- Housing Solutions to join their team. In this role, you will play a vital part in helping individuals with multiple and complex needs prevent a return to rough sleeping. You will provide early intervention, ongoing support, and guidance following discharge from the hospital.
What you'll be doing:
- Offering guidance and developing support plans to ensure clients sustain their quality of life
- Engaging with various support agencies to promote personal independence and well-being
- Acting as the main point of contact for casework within the accommodation, coordinating solutions for drug and alcohol, domestic violence, and mental health issues
What we're looking for:
- Demonstrable experience in case planning and complex case management
- Sensitivity to the diverse needs of individuals from a range of backgrounds
- Strong communication skills, both written and verbal
- Knowledge of local social and health care services and the ability to navigate systems and pathways
Qualifications and skills:
- Experience working independently and providing high-quality care
- Proficiency in Microsoft Word, Excel, and Outlook
- Understanding of the wider issues impacting homelessness and the challenges faced by individuals with complex needs
