This role will coordinate the Staying Well Eastbourne service which is an innovative service offering out-of-hours support to people experiencing acute distress due to their mental health difficulties.
This psychologically informed service follows a crisis caf model and is a model that we deliver across East Sussex.

Main Duties
Monitoring orders and orders enquires inboxes.
Dealing with new orders, print orders and check the details, dates and pricing.

Act as main care coordinator overseeing assigned groups of service users, providing: screening; asset mapping; comprehensive assessments; recovery planning (using motivational interviewing techniques) and reviews of care.
Research, design and prepare subject matter and materials for groups/pods.
Carry out comprehensive service user assessments, including asset mapping of community resources and develop individual recovery plans that clearly identify how achieving each goal will enable progression in addressing substance use and re/integrating into the community.
